Annual Sewage Sludge Report Form

A requirement to complete and submit to the Ohio Environmental Protection Agency an Annual Sewage Sludge Report appears in Part II of most National Pollutant Discharge Elimination System permits issued to Publicly Owned Treatment Works. Such a report on sewage sludge use or disposal includes a summary of the Monthly Operating Report data for the previous calendar year, and is due to Ohio EPA by January 31. One copy of the report should be sent to the Ohio EPA Central Office, and one copy to the appropriate Ohio EPA District Office. Please type all information in the form.
