Rules

• On February 15, 2012 final revisions to OAC 3745-42-05 were adopted.  The effective date of the new rule is March 1, 2012.  

Forms

Updated forms are on the PTI Forms page. Use these new forms from this time forward. An application received at Ohio EPA after May 1, 2006 will not be accepted if it is not on the new form.
