Facility Annual Operational Report forms for landfills, transfer stations, scrap tire monofills/monocells, incinerators, and compost facilities are now available on Ohio EPA’s web site.  These reports are due on April 1, 2012, and cover the 2011 operating year.  For additional information, please contact your Ohio EPA District office (Division of Materials and Waste Management) or Maera Flynn at Maera.flynn@epa.ohio.gov or (614) 728-5338.   

The 2011 version of the instructions for the Hazardous Waste Annual Report have been posted.  The forms have not changed but two new Form Codes were added.  This report is required of facilities that generated more than 2200 lbs of HW (or more than 2.2 lbs of acutely hazardous waste) in any one calendar month.  It is also required of facilities with a treatment, storage, or disposal permit.
